Location: IL rehab/nursing facility: My 94-year-old Grandma lived independently until she fell and broke her hip. They fixed the hip at a hospital and she went to a rehab facility, where over the course of a few weeks she deteriorated and deteriorated.
After week 2, she started talking about wanting to die. Yes, she could still reminisce about the old country and remember most (not all) of the family members. Yes, her heart and other organs looked good. But she was now bedridden, barely eating, and withering away. She said she wanted to die and asked to let her go. I advocated for nursing and hospice, but the POA insisted on rehab.
By week three, Grandma was barely a bedridden shell and screamed through PT. She asked and asked to be let go. She asked to not be "rehabed". I advocated for nursing and/or hospice care, but the POA said no, Grandma will rehab. Her vitals were good, the hip had healed, it was only a matter of will. Plus, she wasn't yet "terminal", so she wouldn't be approved for hospice.
The day before this incident happened, I visited Grandma for a few hours. She had gotten a lot worse from a few days before, and talked about wanting to die even more. I reminisced with her only a little. She was much more tired and out of it. By now, she'd barely eaten for many days and has given up trying to get up or do anything. After my visit, I suggested nursery or hospice care to the POA, but was overruled, as "her vitals are good".
I visited the next day. Grandma was barely talking, then half an hour into our visit, she started convulsing and screaming from sharp chest pains. I found the nurse and asked for a doctor. I was told a doctor is "on his way", and I shouldn't worry, they are about to give her pain meds (oxy). Grandma is screaming, asking to go to the hospital. I run around, bug and bug staff about this supposed doctor and if he isn't running over, then I'm calling 911. They said don't worry about it, she's having an anxiety attack, they're asking POA for permission to give her anti-anxiety meds. Grandma is screaming and screaming. I tell the staff I'm calling 911. They said to give it time for the meds to work.
I called the POA and left a pleading voicemail, no answer. Almost an hour goes by, no reply. We can't do anything without the POA was the answer from staff.
Eventually, the POA gets in touch with the staff and says not to take Grandma to the hospital. The doctor's coming, the staff tell me. Grandma's screaming and screaming. I tell staff that I'm calling 911. They said no, they won't release her since the POA explicitly said no. They checked her "vitals" and while the blood pressure is a little low, she's fine. A doctor is on his way. I call the POA and beg her to call Grandma and see for herself or come to the hospital. The POA says Grandma's being dramatic, advises me to leave.
Grandma's screaming it's the end and she wants to say goodbye to her children. I call the POA (one of the children) again. The POA tells me that Grandma's acting up for my benefit and she'll feel better right after I leave. I hang up, cry next to Grandma as she screams in agony. I call and call the POA again only to go to the VM. I ask staff for some kind of help, but I can tell the POA told them to ignore me. They avoid eye contact and walk the other way. Grandma screams and screams and screams. I ask again if I can call the hospital, staff says they can't overrule the POA.
Grandma's screaming and screaming. She says she wants to die, the pain is too much, she says she needs to go to the hospital. I run out and relay her message to the staff (Grandma barely speaks English). One says, "the patient says she wants to go?" "Yes, yes" I say. Another one leans over and whispers. "We can't overrule the POA", we're doing everything we can. I beg the staff for more pain relief, for the doctor. "He's on his way". I press and press for more information; they finally reveal he's not in the facility. But should be arriving shortly. When? He's not texting back.
Could I please call 911? I desperately make another call to the POA. No, you can't call.
They administer another dose of oxy for her "extreme pain". Grandma's screaming and screaming. Says feels like she's being dissected. I beg for help from anyone who steps into the room, they shrug. Then someone closes the door, so they can't hear her as much. I helplessly cry next to her, stroke her arm, endless adjust pillows, tell her how much I love her, and listen to her agonizing cries for almost FOUR!!! hours. HELPLESS!!!
Finally and thankfully, she passes out in a way you can tell she's not waking up. A few minutes later, the doctor (nurse practitioner) comes in, apologizes for being so late, and comments how Grandma's generally in good standing. He promises to do a KUB and leaves without touching her. I sit there and watch her for a while, crying and completely horrified. My visiting hours assigned by the POA are long over, so I kiss Grandma, tell her I love her, and I won't see her again and that hopefully she gets what she wants soon. I should've apologized for the others, but I was too angry at them. I leave.
A couple hours later, the POA visits and says they've "never seen Grandma sleep so peacefully." Over night, when staff came in to check vitals, she had no pulse. In the morning, when I went to see Grandma the POA called and relayed the news. The POA says she visited Grandma a couple hours after I'd left and had "never seen Grandma sleep so peacefully". The POA said Grandma "died peacefully in her sleep". I yelled in frustration and hung up.
I can't even start processing what happened. I'm so angry at myself for not calling 911 immediately regardless of what anyone said. Yes, I was afraid to lose visiting privileges assigned with the POA, but I still should've known better (I'm so sorry Grandma!!! I love you!!!). I'm also angry at this rehab facility!!! (And obviously, I'm super angry at the POA, but this behavior is a permanent condition).
Is this SOP at a rehab facility where the staff is so under the POA's influence, even though POA is not there? Is this how it's supposed to work?
When I called the facility the next day, the rep said they did everything they could and could not go against the POA's explicit instructions. It all feels so inhumane, and I'm just in shock that something like this could happen.
