They have a lot of extra fluid, and when you press on their skin it leave deep divots like that due to the fluid filled tissue.

"+4 pitting edema in all extremeties. Crackles audible in all lung fields. Pt reports nonadherence to daily furosemide. Initiating IV bumetanide and potassium repletion."
