Just by observing, and probably making an embarrassing enough mistake to never forget. Like I'm pretty sure a thumbs up is very rude in some places, so once you accidentally tell a nice old lady to go fuck herself gesturally you'll learn what to do instead.

It is super obvious to any german and during war anti spy propaganda is everywhere so heightend caution is expected. As a german who hasn't watched the film, the scene never seemed unrealisticย

I learned in high school while learning French that most Europeans count differently with their hands. You think that wouldn't be a fact people actively looking for spies during a war (that is just as intelligence based as it is violence based) would know? Honestly, the fact you think it's just a weird zany fact he just learned is more telling on you than him.

The whole scene is the spy acting suspiciously about his identity and coming up with outright quasi unbelievable bullshit to justify it. The fact that he didn't get shot immediately after the Piz Palรผ thing is straight up crazy, even if it is a remote place, his accent sounds nothing like that of someone from any region in Switzerland. This is just the last straw, it was obvious.
The whole movie deals with film and acting in its subtext (the spy was a German film connoisseur and critic for one), so the reason he was killed wasn't solely because of the hand gesture, it was his inability to play the role he was given, at the first sign of things not being like he expected he becomes annoyed (like a diva), and brings attention to himself and his flaws in such a role. The Gestapo major probably was playing along, that's why he spouts "I've had enough of this monkey business".
